Top Resident Paper Winner: Comparison of Internal Fixation Techniques of Patella Fractures Using Headless Compression Screws vs. Headed Compression Screws and Tension Banding: A Biomechanical Study, Christine Stairs, MD – Louisiana State University – Shreveport Summary: Five different types of fixation were used to fix simulated transverse patella fractures in a cadaver model and ran through 500 cycles of flexion and extension at a rate of 0.125 Hz using an Instron E3000 mechanical testing frame. The fracture gaps were measured at 1, 100, and 500 cycles using Optotrack 3D sensors. The five types of fixation included: Two headless compression screws, two headed compression screws, two headless compression screws plus tension banding, two headed compression screws plus tension banding and K-wire fixation with tension banding. Results show headless compression screws with tension banding having superior fracture fixation compared to other forms tested. K wire fixation with tension banding had the largest fracture gap at 500 cycles. The purpose of this study was to find a superior form of fracture fixation to allow for earlier range of motion in post operative patients. The headless screw fixation could lead to lower rates of post operative hardware removal due to hardware irritation.
